Terms and Conditions of Service
PT Prodia Widyahusada Tbk (“Prodia”) and the Health Service Customers (“you,” “your”) must comply with all applicable laws and regulations. By accessing Prodia’s Payment by Contact Center, you are deemed to have read, understood, acknowledged, accepted, and agreed to all terms and conditions on these Terms and Conditions of Service. Therefore, you must read the Terms and Conditions of Prodia’s Payment by Contact Center carefully to ensure you fully understand them before using Prodia Services.
Definition
- Chatbot contains artificial intelligence features (AI) that aims to mimic human conversation or online chat through text message, website, or mobile application.
- Doctors are medical graduates and health professionals who practice medical treatments and own medical licenses (SIP) to serve in Prodia health service.
- Home service is a program where the nearest home service staff visits a customer for sampling.
- Credit Card is an online or cashless payment method in the form of cards issued by a public or private bank.
- Consultation involves a dialog between patients and doctors to seek medical suggestions.
- Payment Service by Contact Centre is a service provided by Kontak Prodia to accommodate the needs of customers who want to make orders and payments via voice call and live chat methods.
- Live chat with doctors is a virtual means of communication between doctors and patients through an application.
- Test is an examination, panel, or package available in Prodia..
- A Doctor’s Referral is a written request from a Doctor that includes a patient’s tests, panels, or packages that should be conducted in Prodia Clinical Laboratory.
- Voice Call is a telephone call service.
General Terms
- These terms discuss Payment by Contact Center with the following service limitations:
- Payment by Contact Centre is limited to voice call channels and live chat through a chatbot.
- Payment by Contact Center is limited to orders and payments from customers who have referrals: (a) Prodia doctor referral codes and (b) forms from personal doctors/partners. The referral documents must be brought to the test.
- These terms between you and Prodia will be legally binding, and by making orders and payments by Contact Center, you agree to the statement set out in the terms and conditions.
Terms of Service Membership
To access the service, you must master the basic skills of internet use, relevant payment services, and general security. You are expected to possess the internet skills and knowledge required to use the Service.
Legal Capacity
You represent and warrant that you possess the legal capacity, are a minimum of 21 (twenty-one) years old, or are legally married. You possess the legal rights and capacity to enter into these Terms of Service, enroll in the Service on behalf of yourself, use the Service following the Terms of Service, and establish a legally binding agreement in compliance with the rules thereunder.
You warrant that you possess internet skills for ordering services and payments and are able to operate Voice Call and live chat on Contact Prodia.
You shall meet the requirements to be considered as a person of “Eligible Age” to access this Service, being at least 21 (twenty-one) years old or legally married. If you are under 21 (twenty-one) years old and under custodianship, the parent, guardian, or custodian shall agree to these Terms and Conditions.
Privacy
You will be asked to provide your personal data such as full name, address, city, postcode, date and place of birth, gender, ID card or passport (for foreign citizens), and phone number. You affirm the accuracy of the content or information provided to the Service.
Prodia highly values and prioritizes your privacy. We warrant the privacy and/or protection of your data.
Prodia will:
- Keep the confidentiality, completeness, and availability of your Personal Data managed in the Service;
- Ensure that your Personal Data will be obtained, used, and utilized with your permission unless defined otherwise by laws and regulations; and
- Warrant that the data will be used and disclosed with your permission as the owner of the Personal Data, and in accordance with the purpose stated during the data acquisition process.
An exception to this part is condoned if needed legally by Laws and Regulations or Court Decisions with permanent legal force requiring Prodia to disclose your personal data.
Referrals
Referrals are letters issued for patients to get more competent treatments and further measures from health facilities or health professionals.
Patients should include the referral when booking a test at Prodia laboratory and present it during the test, whether conducted at the branch lab or through Home Service.
A referral includes hard copies and soft copies.
Prodia provides patients with doctor consultations at Prodia branches (offline) and live chat (online) for medical suggestions and referrals.
Referring Doctor
You will be asked to enter your Referring Doctor’s Name when you access the Service.
You shall ensure that the Referring Doctor’s Name entered into the Service is correct.
Fees and Types of Tests
The Fees and Types of Tests adhere to those specified on Payment by Contact Center. This applies to both self-administered tests and tests recommended by a referring doctor, with amounts denominated in Indonesian Rupiah.
Selecting a Type of Test
- You are responsible for selecting clinical specimen examination service in Our Clinical Laboratory via Payment by Contact Centre.
- You warrant that your request for the type of tests is correct and aligns with the provided referral.
- You are aware that all selected and paid tests are non-refundable.
- You acknowledge that Article 8 of the Regulation of the Ministry of Health Number: 411/MENKES/PER/III/2010 on Clinical Laboratories stipulates that Clinical Laboratories shall only conduct clinical specimen examination services upon a written request from state or private health service facilities, Doctors, Dentists for oral and dental health examinations, Midwives for pregnancy and pregnant mother’s health examinations, or government Institutions for law enforcement purposes.
- You act in good faith and warrant that the data of the type of test selected is based on a written request from the authorized Party in compliance with Article 8 of the Regulation of the Ministry of Health Number: 411/MENKES/PER/III/2010 on Clinical Laboratories Number 14 of 2021 concerning the Standard of Business Activities and Products in the Implementation of Risk-Based Business Licensing in the Health Sector.
- You assume full responsibility for any legal consequences if the clinical specimen examination service selected through the Service is not based on a written request from the Party appointed to conduct health service to you as stated in Article 8 of the Regulation of the Ministry of Health Number: 411/MENKES/PER/III/2010 on Clinical Laboratories Number 14 of 2021 concerning the Standard of Business Activities and Products in the Implementation of Risk-Based Business Licensing in the Health Sector on Clinical Laboratories and will not file any form of claim in any way toward Prodia.
- You acknowledge that as of April 1st, 2021, Prodia is in full compliance with the Regulation of the Ministry of Health Number 14 of 2021 concerning the Standard of Business Activities and Products in the Implementation of Risk-Based Business Licensing in the Health Sector Chapter 29 on the Clinical Business Standard and Chapter 38 on the Medical Laboratory Standard. You and Prodia, as the service provider, therefore, shall understand any legal consequences and obligations based on the Regulation of the Ministry of Health Number 14 of 2021.
Service Procedure
- You reserve the right to obtain the service in your selected Prodia Branch after conducting registration and payment by carrying the payment slip.
- Service is valid through the date specified in the payment slip.
- Prodia will confirm the accuracy of your identity by asking you to provide your valid ID card.

Electronic Payment
Prodia is committed to creating a Website that is as accessible as possible for all Users, including persons with disabilities. Your browsing experience can be improved by changing the settings on your electronic device (such as changing the text size) or by installing additional assistive technologies. If you have any questions about accessibility, kindly contact Prodia.
Apart from payment transactions, the Service will not utilize and disclose your payment information to the Third Party unless mandated by Laws and Regulations or Court Decisions with permanent legal force. Information given will not be saved longer than needed for service provision.
Service cannot assist you in requesting discontinuance of Credit or Debit Card scheduled payments. In the case of such a request, you shall immediately and directly contact your Credit or Debit Card issuing Bank.
Payment Regulation
Every payment shall be made in advance before benefiting from the Service unless there has been a cooperation agreement signed between you and Prodia. Payment is made using one of the methods stated in Payment by Contact Center, namely Credit Card (Visa, Master, and JCB), Virtual Account (Mandiri, BNI, BRI, CIMB Niaga, Permata, Danamon), and any other legal payments that will be developed.
Payment is deemed valid when a customer receives notification via email confirming that there has been a status change from “Waiting for Payment” to “Paid” and the Prodia bank account has already received the payment.
Rights to Amend Terms and Conditions
You agree and confirm that amendment of Terms and Conditions may be conducted anytime without prior notice. You assume the responsibility to update your personal data in the Service periodically. Any revision or amendment will bind and become effective immediately after the publication of the new terms and conditions or any amendment to the Service provision on Prodia Mobile. Unless notified by Service, Service may use and modify other Service-related terms.
You agree and confirm that amendment of Terms and Conditions may be conducted anytime without prior notice. You assume the responsibility to update your personal data in the Service periodically. Any revision or amendment will bind and become effective immediately after the publication of the new terms and conditions or any amendment to the Service provision on Prodia Mobile. Unless notified by Service, Service may use and modify other Service-related terms.
Prodia may freely transfer, appoint, or delegate all or part of the Terms and Conditions and the rights and responsibilities therein to the desired party. These Terms and Conditions will bind and apply to the beneficiaries, proxy holders, and proxy of the Parties.
Notification
Prodia may send you notification via the email address provided by you during registration, along with a general notice on the Prodia Mobile App.

Indemnification
You agree to defend, indemnify, and hold harmless Prodia, its personnel, employees, and directors from and against any and all claims, damages, liabilities, losses, costs or debts, and expenses (including but not limited to attorney’s fees) arising out of:
- Your use and access of Service;
- Your violation of these Terms and Conditions;
- Your violation of the rights of the third party, including but not limited to copyrights, property, or privacy rights; or
- every claim that one of your Status Proposals incurs damages to the third party.
- Defense and indemnification obligations shall survive based on the Terms and Conditions and the use of the Service.
Governing Law
Every activity carried out under Payment by Contact Center is legally protected and based on the Regulation Number 11 of 2008 as last amended by the Regulation Number 19 of 2016 on Electronic Information and Transactions, the Regulation of the Government Number 71 of 2019 concerning the Electronic System and Transactions, and the regulation of implementation and other regulations, along with the amendments conducted at any time, and toward any form of binding arising out the use of Payment by Contact Center, have fulfilled the rules and requirements for the validity of an agreement based on the Civil Code.
The Parties agree that these Terms and Conditions comply with the law of The Unitary State of the Republic of Indonesia.
