Definition

1. Data is your personal information obtained, collected, processed, analyzed, displayed, opened and/or all forms of management related to data or information that you provide to the Application or collected from you, both when you register in the Application, access or use the services in the Application.
2. Abuse acts are one or more of the following acts:
1. Actions that constitute violations of applicable laws and regulations, fraud, embezzlement, money laundering, financing of terrorist activities, payment for illegal goods and/or services, prostitution activities, or pornography/pornoaction, gambling, or other acts that constitute a violation of the interests of any party.
2. Actions that are detrimental or may cause losses to Prodia.
3. Actions committed for personal gain, or performed abusively, falsely, or fictitiously for personal gain.
4. Actions taken by hacking, tricking, deceiving, and/or manipulating the Application, Application Services, or systems operated by the Application or systems operated by other parties cooperating with Prodia, and/or Prodia' s affiliates.
3. PT Prodia Widyahusada Tbk (Prodia), a company established under the laws of the Republic of Indonesia engaged in health services.
4. Prodia courier Mobile Application (Application) is an application built to facilitate Prodia Courier Officers in picking up referral samples from Customers.
5. The courier (you) is a special officer in each Prodia Branch, whose duty is to pick up the Customer referral sample.
6. The customers are hospitals/clinics/laboratories that have a cooperative relationship with Prodia.

User Consent

By using the Application, you have read and understood this privacy policy and you have given your consent as the owner of Personal Information to the matters stipulated in this Privacy Policy as a legally binding agreement, and represent and warrant that you are an adult according to applicable law, or at least 21 (twenty-one) years old or legally married. You hereby represent and warrant that you have the right, authority, and capacity to use the Application.

The Scoop of Privacy Policy

This Privacy Policy governs the Utilization of Personal Data. By using the Application, you are deemed to have read this Privacy Policy and agreed to the mechanism for Utilization of Your Personal Data as stipulated in this Privacy Policy. If Prodia asks you to provide Customer information when using the Application, then such information will only be used for the purposes of providing the Service in accordance with this Privacy Policy. Prodia has the right to change, delete and apply the new provisions of this Privacy Policy at any time. You are expected to check this Privacy Policy page periodically for such changes. By using the Application service after such changes, you are deemed to have acknowledged and agreed to the changes to the provisions of this Privacy Policy.

Registration

1. You are required to register to be able to use the following Application with the features contained in it.
2. To register in the Application, you must provide the information Prodia needs as stated in Point 5 below.

Personal Data

1. Prodia collects your information when you create an account and profile, fill in your data or put other information in the Application.
2. Information regarding personal identity that must be filled in by you when creating an account in the Application with the aim of providing better services for the purposes of Application functionality is full name, phone number, and email.
3. Prodia also has the right to collect information when you use the Application, including but not limited to: GPS location/coordinates (if you activate this feature), Internet Protocol (IP) address (if using a browser), operating system, browser type, referring Application address and your activities in the Application, Files and Camera access. You may enable or disable location recognition services while you are using the App.
4. Prodia has the right to verify directly to you the information regarding your personal data submitted through the Application.
5. By using the Application, you warrant that the information you provide is accurate and correct.
6. If the information you provide is found to be incorrect, Prodia shall not be responsible for any consequences that may occur in connection with the provision and use of the incorrect information.
7. You guarantee that the information you provide is accurate and true, so Prodia has the right to verify directly to you the information regarding the Customer's personal data that has been submitted by you through the Application.
8. If the information you provide is found to be incorrect, Prodia shall not be responsible for any consequences that may occur in connection with the provision and improper use of the Customer's information.

The Purpose of Collection of Personal Data

Prodia uses personal information, either in part or in whole, that Prodia obtains & collects from you, with the following objectives:

1. Provide information to the Customer for the sample pick-up officer who will come to the Customer's location.
2. Provide information to Customers regarding the status of sample pick-up orders.
3. Use and disclose your updated location information to customers.
4. Use your information to contact you if necessary.
5. Anticipate and resolve problems related to services provided to customers.

Collection of Information Everytime You Use the Application

You can provide Prodia with technical data and Location every time you use and/or visit the Application. Every time you access through your mobile device, Prodia will be able to track and collect your geographical information in real-time. In using the Application, you may be asked to activate the Global Positioning System (GPS) on your mobile device so that Customers can find out your position information in the sample pickup process and its updated status. In addition, each time you use and/or visit the Application, certain information may also be collected automatically using cookies. Cookies are small application files that are stored on your mobile device. Prodia uses cookies to track your activities to improve Customer Service. Most Mobile Devices and internet browsers support the Use of cookies; but you can adjust the settings on your Mobile Device or internet browser to refuse certain types of cookies or certain specific cookies. Your Mobile Device and/or browser also allows you to delete any cookies that have previously been stored at any time. However, it may affect the functions available on the App.

Access and Permission on Your Device

For your Android and iOS devices, the App will need permissions to:

1. Internet access and network state
The Application requires an internet connection, so the Application monitors the condition of your internet and network to be able to provide notifications, if there is a lost-connection at any time that results in the Application cannot run (optimally).
2. Camera access
The Application requires you to include photos in some parts of the Application, you can use Camera as one of the media to be able to take pictures. Granting access to the Camera on the device serves for you to take pictures in choosing your profile photo, attaching photos of files / samples taken. Granting access to the application to use the camera and / or photo gallery or storage media on your device is "READ_EXTERNAL_STORAGE" & "WRITE_EXTERNAL_STORAGE" access to read and save photos / images, with the possibility of being temporary (the results of processed files specifically for this application can disappear by themselves).
3. Photo access, gallery, and files
The Application requires you to include images and/or documents in some parts of the Application, you can look at the gallery and/or file browser to find photos, images or related files as needed. The application requires you to be able to store the data generated from the application side. Granting access to photos, galleries and files for "READ_EXTERNAL_STORAGE" & "WRITE_EXTERNAL_STORAGE" access on your device, serves for you to select a profile photo and/or select supporting documents needed in the sample pick-up service. 11)Granting access to applications to read, write and process files on storage media on your device, may be temporary (the results of processed files specifically for this application can disappear by themselves).
4. Location Access
You can view the permissions used by the Application and disable them at any time through the settings menu. Granting access to the application to use your current location data when this application is used, namely "ACCESS_COARSE_LOCATION" & "ACCESS_FINE_LOCATION"Granting location access is used to make it easier to determine your current location with the customer's location for sample pick-up service facilities.

Personal Data Disclosure

1. Prodia shall have no obligation to maintain the confidentiality of your Customer Data, in the event that (i) it is already known to the public, (ii) it is known by Prodia from other parties without any breach of confidentiality (iii) required by applicable laws and regulations (iv) required by legal process.

3. To maximize the quality and optimal service experience, Prodia has the right to have accurate and true information. Security and Protection of Personal Information

Prodia has made every effort to maintain the security of your information and prevent it from being used by unauthorized parties. You are responsible for maintaining the confidentiality of your account details, including phone number, email, password (as applicable), or one-time password (OTP) with anyone and must always maintain and be responsible for the security of the device you use. You will release Prodia from any loss or claim in the event that you fail to maintain the confidentiality of your account details, including password, email or one-time password (OTP), and if you are negligent in maintaining the security of the device you use.

Your Rights to Your Personal Data on the Application

You can access and change your Personal Data related to your email address, telephone number, through PIC Prodia.

Applicable Law

This Privacy Policy is governed by the laws of the Republic of Indonesia and you are required to comply with all applicable regulations in the Republic of Indonesia.

Terms and Conditions of Use on the Application

1. Access to Application
1. You are agree to:
   1. access the application and/or use the Service only for lawful purposes and in a lawful manner, at all times, and also agree to carry out any activity related to the Service or application in good faith;
   2. comply with all and any guidelines, notices, operational regulations, policies, and instructions regarding the use of the Service and/or access to the application, as well as any amendments issued by Prodia from time to time;
   3. warrant that any information or data you publish or display in applications related to the Services is accurate.
2. You acknowledge that you will use this application only for its intended purpose, which is for sample pick-up officers.
3. You will keep confidential and will not misuse the information you receive from using the application.
4. You understand and agree that your use of the application must also comply with Prodia' s Privacy Policy and other rules, as may be amended from time to time.
2. Account closure
"Closure" means the permanent deactivation of your application account. Closure of your account may occur due to one or more of the following:
1. Your account becomes a Dormant Account, and Prodia, based on Prodia' s policy, decides to close your account.
2. Prodia obtains an order from the competent authority, agency, or court or based on the prevailing laws and regulations to close your account.
3. Your account is proven to have committed abusive acts.
4. Your account is proven to have been used to commit an action that is a violation of applicable laws and regulations or a violation of the interests of any other party, OR you are proven to have committed an act that is a violation of applicable laws and regulations or a violation of the interests of any other party.
5. Your account is proven to have been used to commit an action that may harm Prodia and/or any party, OR you are proven to have committed actions that may harm Prodia and/or any other party.
6. Prodia, based on Prodia' s own consideration and policy, decides that the closure of your account must be done for the security purpose of Prodia and/or any other party.
7. You, based on sufficient evidence and information, are declared to have left / not become an active employee of Prodia.

Changes to Privacy Policy

Prodia may change this Privacy Policy at any time based on Prodia' s own consideration and decision without prior written notice. Prodia will record the changes, in which every change to the Privacy Policy will be announced through the Application by explaining the last date of each change to the Privacy Policy, at the top of the Privacy Policy. If you do not want to be bound by the new Privacy Policy, then you may not use the Application further.
